Our Programs

The Focus of Our Efforts

RAISEtown Care Connect provides a variety of programs and services designed to support individuals and families by increasing access to resources, promoting independence, and improving quality of life.

Equipment Loan Program
Provides free durable medical equipment, including wheelchairs, walkers, hospital beds, crutches, and other mobility aids, to individuals with a doctor's prescription. Equipment may be borrowed for as long as it is needed at no cost to the recipient.


Specific Assistance Program
This program offers financial assistance for medically necessary items and home modifications that individuals may otherwise be unable to afford. Requests are reviewed based on eligibility, need, and available funding.


Personal Emergency Response System (PERS)
The PERS Program helps eligible individuals maintain independence and safety at home by providing access to emergency response monitoring services. Assistance is available to help qualified participants obtain and maintain a personal emergency response system.


 

Early Intervention
Our home-based Early Intervention program provides speech therapy and special instruction for eligible children from birth to age three. Services focus on coaching and empowering families to support their child's development through everyday routines and activities.


Occupational Therapy
We provide school-based occupational therapy services to eligible students in the Mount Union Area and Southern Huntingdon County School Districts. Therapy supports students in developing the skills needed for success in the classroom and daily activities.


Community Outreach & Education
RAISEtown Care Connect connects individuals and families with information, resources, and community programs that promote independence, dignity, and well-being. Through outreach events, partnerships, and education, we help raise awareness of available services and supports.
