

Personal Emergency Response System (PERS)

RAISEtown Care Connect (RCC) is proud to offer the Personal Emergency Response System (PERS) Program to help individuals remain safe and independent in their own homes. A Personal Emergency Response System provides immediate access to emergency assistance with the push of a button. Depending on your needs, devices are available for use inside the home or while on the go and may include optional fall detection technology.
Program Transition
For many years, the Huntingdon-Bedford-Fulton Area Agency on Aging (AAA) administered the PERS Program for eligible residents. Due to changes in available funding, AAA is transitioning the administration of this program.
Effective July 1, 2026, RAISEtown Care Connect has assumed responsibility for administering the program, coordinating services, and paying approved PERS providers for individuals who are currently enrolled. Existing participants should experience no interruption in service during this transition.
New Referrals Beginning August 2026
Beginning in August 2026, RAISEtown Care Connect will begin accepting referrals for new participants, subject to eligibility requirements and available program funding.
Individuals may qualify if they:

Live alone or are alone for significant periods of the day.

Have a medical condition, mobility limitation, or increased risk of falls or emergencies.

Would benefit from access to emergency response services.

Have a physician's prescription or other medical documentation supporting the need for a PERS device.
Approval is based on program eligibility and available funding.

For Healthcare Providers and Community Agencies
Complete the PERS Referral Form and submit it to Referrals@RAISEtownCareConnect.org. If available, please include the physician's prescription and any completed intake paperwork to help expedite the referral process.
For Individuals and Families
Individuals or family members may contact RAISEtown Care Connect directly to request services. Complete the PERS Intake Packet and ask your physician to provide a prescription (can be faxed to 814-643-6085) for a Personal Emergency Response System (PERS) device.
Once all required documentation has been received, RCC staff will review your eligibility, discuss available provider options, and coordinate installation with the selected provider.
Provider Options
RAISEtown Care Connect partners with multiple approved PERS providers, allowing participants to select the system that best meets their needs. Available options include traditional in-home systems, cellular systems, mobile GPS devices, and fall detection technology.
